Resistance work on wobble boards, balance pads, and stability balls has been sold on the promise of extra core recruitment for years. A meta-analysis now tests it head-on: five databases searched, seven randomized controlled trials directly comparing unstable resistance training with stable resistance training, six of them pooled for the analysis.
Three outcome families were measured — lower-limb strength (maximal isometric leg extension, Five-Times Sit-to-Stand), mobility (Timed Up and Go, gait speed), and fear of falling (Falls Efficacy Scale-International) — pooled separately for short-term (10 to 12 weeks) and long-term (24 weeks) interventions.
Which side won on strength?
Neither. After short-term training, unstable training showed no significant improvement over stable training on any strength or mobility outcome. And on the Five-Times Sit-to-Stand, stable training trended better — not statistically significant, but the direction was unambiguous.
The reason is mechanical. On an unstable surface the load you can handle drops. What balancing consumes is postural control, not strength, and the loading available to the target muscle is cut by exactly that much. The core does work harder; the prime movers work less.
So what did unstable training win?
- Fear of falling — significantly reduced after short-term training alone. Same strength numbers, less sense of being about to go over.
- Timed Up and Go — significantly better than stable training after 24 weeks of training. Standing from a chair, walking, turning, and sitting back down is a composite task that reflects postural transitions more than raw strength.
What instability trains, then, is not strength but the ability to deploy strength in a real situation — showing up early as a psychological measure and later as a functional one. Worth noting that the long-term outcome needed 24 weeks to reach significance.
How should a big-three lifter place this?
The trials studied older adults, so the numbers do not transfer directly — but the load-versus-stability trade is the same everywhere. Raising a Muscle Index is entirely stable-ground work. Squat, bench, and deadlift maxes come out of conditions that permit maximum load, and instability removes exactly those conditions.
The place for unstable work is accessory time, after the main sets. Single-leg patterns, offset loading, and unsteady footing train balance and postural control, and the results above say those measures genuinely improve. When that time starts replacing main-lift volume, the score goes down.
Seven trials, six pooled — a small evidence base, and entirely in older adults. Whether it transfers to younger lifters is not answerable from this data. What is clear within its range is that the claim "unstable surfaces build more strength" has no support.
